This form is to be filled out by club directors or administrators only.

Use this form to request a Certificate of Insurance (COI).  Insurance is valid for events in which ALL participants are USAV members (does not include non-sanctioned events such as private lessons). Certificate holder is an entity, such as a school district, church, or facility. Individual schools as part of a school district do not need to be listed, just the district name and the district address.

Turnaround time on COI requests is 3-5 days.  For insurance to be in place, club and event must be in compliance with the most stringent health and safety protocols in place, in the locality in which the event is taking place, at the time of the event.  All certificates include excess liability coverage and sexual assault & molestation ("SAM") coverage.

By submitting this request, you acknowledge, understand, and agree that insurance is ONLY in place for CEVA/USAV sanctioned events and is not valid for events sanctioned by another organization or otherwise covered under private insurance policies.  

Your club must be active for the 2023-2024 season for a COI issued after 9/1/2023 to be valid.  COIs will not be issued to inactive clubs.
