DSMI | Transportation Request Form
This form must be completed BEFORE any student is approved to ride the bus.  One form per student.  ALL STUDENTS IN GRADES K-3 MUST BE PICKED UP FROM THE BUS STOP BY A PARENT if they do not have an older sibling (at least grade 4) on the bus.  NO EXCEPTIONS.  Parents must be at the actual stop to release and retrieve students.

ATTENTION PARENTS:
● House stops are NOT authorized unless specifically approved in writing by the Transportation Coordinator. Your child must be at the designated corner stop when the bus arrives.
● Routes are developed based on need and location of students. Stops are not automatically added year to year. Students will not always have a stop close to their home; walking to the bus stop may be required. PLEASE DRESS FOR THE WEATHER.
● The Transportation Department strives to provide a safe ride for all of our students. Students must adhere to all bus riding rules or they risk losing their bus riding privilege.
