Please use this form to:

(1)  tell us about the tree or the sculpture/art work/installation you will contribute to the festival 
(2)  when you will come to set it up and take it down
(3)  give details about participation in the festival events 
(4)  write the text of the display card for your tree /art/installation
(5)  tell us whether you would like to host a 'travelling tree of peace' for 48 hours some time between  Advent
        Sunday 3rd December  and Sunday 17th December - the festival carol service

IMPORTANT  : What will we say about your tree?
Please tell us who you are, where you are based and the inspiration for your tree. Include any details that will be of interest. This will be copied and pasted to form a display card for the tree
