2023-24 MHS Athletic Packet and Certification Process
Dear Parents/Guardians,
We are preparing for the start of our full athletic seasons. Some parents have completed our full athletic certification packet and submitted the various forms. To streamline this process, we have created this certification process that includes all needed forms including:

-Physical Form
-Proof on Insurance
-Acknowledgement and Assumption of Risk
-Concussion Information Sheet
-Sudden Cardiac Arrest Sheet
-Athlete Committed Document

Please complete this process to ensure we have all of the required components to certify your student is ready for the athletic season.

Please review each of the documents listed below and type your full name in each of the questions related to the document. By typing your name and submitting this form you are stating that you have read, understand and agree with the terms and information contained in each of the documents.

Prior to participating in a practice or a competition a parent/guardian must complete this process and provide the school with proof of insurance and documentation of a current physical on the provided physical form.

Step 1 - Parent/Guardian and student must review all documents and enter your name in each of the areas on this form.
Step 2 - Use the MHS Physical Form to take to your doctor for your annual physical (you can get this form in the main office). Turn this form into the office.
Step 3 - Provide the school with your proof of insurance. You can scan or take a picture of it and email it to Amy at abourke@sisuhsd.net
Step 4 - Once we have the information from steps 1-3, we will provide your coach with information that you are certified and cleared to participate in practice and competition. You may not attend or participate in practice or competition until your coach receives the certification that you are cleared.

Acknowledgement and Assumption of Risk - Type your name to indicate you have read, and acknowledge that you understand and accept risk and authorize the student named above to participate in Athletics.
