This peer-evaluation affords you the opportunity to reflect on your experiences with your team members holistically and allow faculty to analyze team performance, learning, and professional development.

For this survey, you will be asked to rate each of your team members individually based upon their performance. To protect confidentiality, students will not receive results of the peer evaluation in teams with fewer than 3 respondents to this survey.
 
Your responses on this evaluation may be shared with your professor/adviser for their review and discussion. Any other use of the information you provide in this form will be presented in aggregate form only and will not include any personal identifying information.
