We are almost there! Camp is just around the corner! This is the final deadline. By completing this form, it helps me expedite the check-in process upon arriving to camp, and saves me from manually inputting over 200 students' names into my computer. :-) so thank you!!
BEFORE BEGINNING THIS FORM:
Make sure to have the following information for each student as each field is required in this form.
1. FULL STUDENT NAME
2. LAST GRADE COMPLETED
3. MALE/FEMALE
4. NAME OF CHURCH ATTENDING WITH
5. CHURCH CONTACT PERSON FULL NAME
6. SHIRT SIZE
This form is meant to be used "per student". So once you have submitted for one student, simply click "submit another response" and complete it for your next student. I know this may be a little tedious to re-enter church name and contact person for each student, but this does truly save my sanity and time as I prepare all these little details that help make camp run a bit smoother! So thank you for your help! I appreciate you!
After you have added all students, I can send you a copy of the spreadsheet this form creates for your reference and convenience, and to double check information. From this spreadsheet, you can add or delete students if needed all the way up until the week of camp!
