Once this is completed you will receive a copy of the forms and our Privacy Notice for your information.

It is very important for Figure of Eight Education to be able to contact Parents/Carers in the case of any emergency or centre closure.
We ask that you keep us up to date with new mobile phone numbers and add any email addresses you may have on the collection data form. Please ensure you have given a valid email address as this will be used to communicate relevant information with you.
