NYS TESOL Request for CTLE Certificates

Continuing Teacher and Leader Education (CTLE) Hour(s)
or
Certificates of Participation for any NYS TESOL webinar/event you have participated in.
Please fill out all the fields below
within 30 days of the event
and
allow at least 4 weeks
for processing.
You will need to complete a new form for each webinar or event that you wish to apply for CTLE Certificate.
The CTLE Form can serve as a certificate of participation.
You must be a current members of NYSTESOL apply for CTLE Certificate.
