This form is to apply for Continuing Teacher and Leader Education (CTLE) Hour(s) or Certificates of Participation for any NYS TESOL webinar/event you have participated in.  
  • Please fill out all the fields below within 30 days of the event and allow at least 4 weeks for processing. 
  • You will need to complete a new form for each webinar or event that you wish to apply for CTLE Certificate.
  • The CTLE Form can serve as a certificate of participation.
