The Philippines Food and Drug Administration (FDA) and The Philippine Women's University-School of Pharmacy is conducting an assessment to evaluate the knowledge, prevailing attitudes and current practices on adverse drug events (ADR) reporting among health care providers involved in the reporting of ADRs. Your responses to this survey will help us understand medication safety reporting habits and improve methods for medication safety monitoring.

This survey will take approximately 15 minutes of your time. We will ask you questions about your knowledge of pharmacovigilance, attitudes and practices towards ADR reporting, and general sociodemographic and professional information. You will not be penalized in any way should you choose not to participate or to withdraw.

Your answers to this survey will be completely confidential. No personal identifying information will be collected during this survey. Your employer will not be informed of your responses or whether or not you chose to participate. We may share our findings in publications or presentations. If we do, the results will be aggregated and no statements, declarations or specific findings will be attributed to you. There is always a risk of data being intercepted or accessed by anyone when you share information online. We are minimizing this risk by using a secure system for this survey, collecting only anonymous data, and storing all electronic data on a password-protected, encrypted computer.
