PLEASE READ CAREFULLY:
The comprehensive ADHD assessment takes AT LEAST TWO scheduled clinic appointments to complete.
It will comprise of a detailed history taking & examination, the gathering of additional supporting documents, completion of questionnaires, & medical investigations (blood test, ECG, and urine drug screening).
Undertaking this assessment does NOT guarantee a diagnosis of ADHD, nor the prescription of medications for ADHD. Medications will NOT be prescribed until the full assessment is complete. It does, however, guarantee a thorough mental health assessment and advice regarding treatment options for mental health concerns, irrespective of diagnosis.
Treatment of ADHD is structured and based on both medication and non-medication strategies. MEDICATIONS FOR ADHD are HIGHLY REGULATED and require strict conditions to be met to qualify for authority approval. These conditions are set out by State Government regulation and must be adhered to by the prescribing doctor. Certain pre-existing conditions may affect the suitability of certain medications & even preclude their use. Once prescribed they require close monitoring, particularly during the initial stages of treatment.
The presence of certain pre-existing psychiatric or medical problems (such as psychosis, active depression, significant anxiety, cardiac conditions or neurological conditions) may require special authority to be sought IF the prescribing of ADHD stimulant medication is to be considered. This usually requires a SECOND OPINION, from another independent psychiatrist or relevant specialist, a comprehensive report and an application to the NSW Pharmaceutical Regulatory Unit, all of which may attract separate fees.
ALL patients will be required to undertake a urine drug screen prior to commencing stimulant medication. This is a requirement to ensure that no illicit drugs have been consumed in the recent period. If you wish to streamline the assessment process, please attend with results of recent tests including blood tests (FBE, UEC, CMP, LFT, TFT, B12/Folate, Iron studies, Vitamin D), ECG and Urine Drug screen.
Non-stimulant ADHD medication treatments are also options that do not require this process.
The following requirements
MUST be met in order to be offered an ADHD Assessment with Dr Shen:
1. You have
NO prior history of significant illicit drug use or significant alcohol use (where 'significant' is defined as regular or frequent use of a drug that resulted in you developing signs of an addiction or dependence on the drug; any intravenous drug use in the past; or having received prior treatment for a drug addiction).
If there is a past history of significant drug use or alcohol use
, you will need a second opinion by another independent psychiatrist for any stimulant treatment. If you have been using any illicit drugs in the preceding 3 months, you cannot be prescribed any stimulants. This also applies to prescribed or non-prescribed medicinal CBD or THC.2. If you have a prior diagnosis of ADHD and have been on treatment in the past 2 years, you will need to provide a letter of handover from your previous psychiatrist or paediatrician in order for Dr Shen to continue prescribing for you. If your diagnosis and treatment occurred prior to two years ago, you will need to provide a prior SPECIALIST letter which outlined your diagnosis and treatment at that time (a GP letter will not suffice). The transfer letter should specify the assessment process, and include collateral sources of information, otherwise this will not suffice.
3. You are willing to assist in providing evidence of your ADHD symptoms prior to the age of 18, which includes EITHER providing copies of your school reports OR having a close family member available to complete a questionnaire based on their observations during your childhood. (Note: it is a necessary requirement for an ADHD diagnosis that symptoms are shown to have begun in childhood).
4. Please note, if there is a history of certain conditions, such as psychiatric conditions like Bipolar Disorder or psychotic symptoms; or a family history of this, or certain medical conditions such as heart conditions, or epilepsy, there may be additional investigations required, or referrals to relevant specialists. In some cases, there may be restrictions to specific types of treatments as well.
