WVFFI Site Visit Intake Questionnaire
The West Virginia Forest Farming Initiative (WVFFI) uses this questionnaire to schedule forest site visits. Please fill out the following to the best of your ability if you are interested in cultivating forest botanicals (i.e. ginseng, golsenseal, black cohosh, ramps, etc.) and/or other Non-Timber Forest Products (gourmet and medicinal mushrooms, maple syrup, etc.). The more information that you can provide us, the better we will be able to assist you in your forest farming endeavors and connect you to appropriate markets. We promise, this questionnaire should only take a few minutes!

*Applicants must be West Virginia landowners or long term lease holders (minimum 5 years, some species may require longer lease commitment) with suitable habitat.

*The grant award will be contingent upon a site visit by WVFFI or documentation confirming suitable habitat.
