LYSA 2023 FALL SCHOLARSHIP REQUEST
This form is required for scholarship requests for the Fall 2023 Season. Scholarships are given on a first come first serve basis (as available funds allow) in the amount of $45 (recreation) unless extraordinary financial hardship can be demonstrated.  This is intended to help the club serve the most players possible in our community.
