This form is to register your child for Little Farmers Day Camp July 8th -12th or July 15th-19th

9:00 am - 12:30 pm. Ages: 4-6

Please complete the following information about your child. This information is confidential and kept on file in case of an emergency. This form must be completed before your child can attend camp. The cost is $140.00 per camper. Payment is required for registration. Registration is not complete until payment is received. Refunds are NOT guaranteed

All campers must be restroom independent.
