This form is to register your student for the Middle School Hang Out Club.  The lead HST will contact you to provide you with a Zoom link to participate the day before the club is scheduled.  Please complete this form for each student in your family wishing to participate. By filling out the survey you are granting permission for your student(s) to participate in the Middle School Hang Out Club. Our club is open to grades 6-8. This club meets on 2/9, 3/9, and 4/13 at 10am.

In this club, your student will be able to: hang out with other middle schoolers, show and tell what they have been learning, and discuss and share the qualities of being a good friend and how to make friends.
