Waiver for tryout: PARENT/GUARDIAN AUTHORIZATION AND WAIVER OF LIABILITY To introduce the Ramball Softball Club team to accept registration and permit participation in RSC by the named individual, I the parent or guardian of said individual hereby give my consent and agree to release, indemnify and hold harmless RSC, it’s coaches and representatives, from any claim arising out of injury to the above named individual except to the extent and in the amount covered by any liability insurance held by the RSC. I also agree to hold harmless RSC, it’s coaches and representatives from any claim arising out of injuries or conditions caused or aggravated by my refusal to obtain available medical treatment. *
