HJTC Travel Subsidy Request Form 
Travel Subsidy Policy:
The Hilo Junior Tennis Club will subsidize $50 for travel costs for no more than one West Hawaii or off-island tournament per quarter (i.e. January to March, April to June, etc.) for each qualifying junior per year. 

Deadline: Must apply within 30 days following the quarter requesting reimbursement to Junior sanctioned USTA/HPS tournaments. If you have any questions, please email Kawika Uyehara at kawikauyehara@gmail.com.

A. Applicant is a current club member for at least six (6) consecutive months immediately prior to the date of travel.
B. Agree to play the tournament to its completion representing the Hilo Junior Tennis Club. Exceptions will be considered due to injury or other special circumstances on a case by case basis.
C. Must be a USTA/HPS sanctioned junior tournament held in the State of Hawaii.
D. Applicant or parent participates as a volunteer in at least one club drills, Match Play, or club sponsored tournament, or is a Board member, chairperson or committee member of a club sponsored activity during the quarter that the travel subsidy is being requested for.
Juniors and parents are strongly encouraged to participate in club activities.  Please check the items as applicable and provide date volunteered in the quarter the subsidy is requested for:
Volunteered at drills picking up balls or drilling members or monitoring Match Play.
