**  Discontinue form must be submitted BEFORE the 20th of the month prior to your final month

JAN   - submitted by Dec 20
FEB    - submitted by Jan 20
MAR  - submitted by Feb 20
APR   - submitted by Mar 20
MAY  - submitted by Apr 20
JUNE - submitted by May 20
JULY  - submitted by June 20
AUG   - submitted by July 20
SEPT  - submitted by Aug 20
OCT    - submitted by Sept 20
NOV   - submitted by Oct 20
DEC    - submitted by Nov 20

**  FMP's are non-refundable.  They will be held in credit until you return

     Once you have completed the form you will be contacted within 24-48 hours via email or text to close out your account.
