Spay/Neuter Request Form
Eligibility: Pet owners that reside within the East Side Health District (zip codes 62201, 62203, 62204, 62205, 62206, and 62207) may be eligible for free spay/neuter services. If the pet owner resides outside of our service area, we are unable to accommodate requests at this time.  You can refer them to Carol House Quick Fix Clinic.

Our package includes free spay/neuter, vaccines (rabies & DAPP), microchip, nail trim and dewormer. We also provide a cone if needed. We provide transportation to and from the vet clinic.

Make sure names are spelled correctly (ask client to spell if you are unsure) and address and phone numbers are accurate (repeat back to client).

Note: "Email address" below is the email address of the pet owner. A receipt will be sent to the email address on file. Individuals without an email address can use formmanager@gatewaypets.org.

What happens next? (Make sure to share w/ Clients)                                                  
Once this form is submitted, you will be added to the waiting list. GPG has a waiting list and it can take up to 6-8 weeks before we have an available appointment; but oftentimes it is more like 3-4 weeks. You will not hear from us until we have an appointment available for them. We will contact you when it is your turn on the waiting list and we have an appointment available for you and discuss pickup date, time and details. If your phone number changes or you have not heard from us within 6 weeks, please call 618-687-8007 Ext. 5. We provide this service for free to any resident living in the East Side Pet District, but as a non-profit organization, we always welcome donations. Do you have any questions for me?
