The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious. The state of medical knowledge is evolving but the virus is believed to spread from person-to-person contact and/or by contact with contaminated surfaces and objects and through air transmission. People reportedly can be infected and show no symptoms and therefore spread the disease. The exact methods of spread and contraction are unknown, and there is no known treatment, cure, or vaccine for COVID-19. Evidence has shown that COVID-19 can cause serious and potentially life threatening illness and even death.

COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal, state, and local health agencies recommend social distancing and have, in many locations, prohibited the congregation of large groups of people. There are also many regulations in place with regard to sanitization of commonly touched surfaces and personal hygiene guidance. Chabad Student Center North Orange County. ("Chabad") has put in place preventative measures at the guidance of the California State Department of Health to reduce the spread of COVID-19. However, Chabad cannot guarantee that you will not be exposed or become infected with COVID-19. Preventative measures include, but are not limited to: requiring all students and staff to wear face masks while on location, with the sole exception of being alone in an office or assigned area, and when eating or drinking, making hand sanitizer available to students, faculty, staff, and visitors, regularly cleaning and disinfecting commonly touched surfaces, reduction in capacity, communicating regularly about social distancing guidelines, requiring masks to be worn when working in small groups, requiring hand washing and personal hygiene be adhered to at all times. Policies are also in place that require anyone who is experiencing symptoms of the coronavirus should not come to Chabad.

Chabad cannot prevent you from becoming exposed to, contracting, or spreading COVID-19 while utilizing or participating in Chabad’s services, programming or premises. It is not possible to prevent against the presence of the disease. Therefore, if you choose to utilize and/or participate in Chabad’s services or programming and/or enter onto Chabad’s premises you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.

ASSUMPTION OF RISK: I have read and understood the above warning concerning COVID-19. I hereby choose to accept the risk of contracting COVID-19 for myself in order to utilize and/or participate in Chabad’s services or programming and enter Chabad’s premises. These services are of such value to me that I voluntarily accept the risk of being exposed to, contracting, and/or spreading COVID-19 in order to utilize and/or participate in Chabad’s services or programming and premises in person.

WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against Chabad and its owners, officers, directors, managers, officials, trustees, agents, employees, or other representatives in connection with exposure, infection, and/or spread of COVID-19 related to utilizing and/or participating in Chabad’s services or programming and premises in person. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.

CHOICE OF LAW: I understand and agree that the law of the State of California will apply to this contract.
