Dear Parents,

Please take a moment to complete this form to let us know whether your child will continue at St. Eugene School for the 2021-2022 academic year. Spaces that are not reserved in writing by current families will be made available to new students. Please check below (this form will also be available in the office, on our website, and on Schoolspeak).

This form will help us plan for the coming year, and as we receive applications from new families, but please remember your child's seat is not guaranteed until your 2021-2022 registration fee has been paid. Thank you St. E Family!

Yes, my child(ren) plan to return to St. Eugene School for the 2021-2022 academic year. I realize that space is not guaranteed until my re-registration fee has been paid and my financial account is current.

I will make a $100 deposit per child (Zelle @St Eugene School, ckingsby@steugene.net, or call office to make a cc payment) toward the $300 re-registration for the 2021-2022 school year. The balance must be paid in full on or before Friday, May 21, 2021.

I will make a $275 payment per child (Zelle @St Eugene School, ckingsby@steugene.net, or call office to make a cc payment) for early re-registration for the 2021-2022 school year (a $25 discount is applicable if paid by Friday, April 23, 2021).

No, my child(ren) will not be returning to St. Eugene School next year. I understand that all accounts must be settled in full before school concludes in June, or school records, including report cards, will not be released.
