I, the undersigned, declare that:
(1). I am the parent of a dancer below the age of 18

(2). I agree to the audiovisual recording of the dancer's participation and do hereby authorise Polanie to use my child's name in any recording 

(3). I grant Polanie all necessary rights to use any recordings and to authorize the use thereof for and in connection with Polanie promotion, the communication to the public by telecommunication, (including communication via Internet), the distribution, the use and the promotion of the recordings as well as the sale and use of excerpts therefrom, in all languages, by any and all means currently known or developed in the future, throughout the world, without any term, format and market restriction.

(4). The above authorisations are deemed granted free of charge and vested in to Polanie.

(5). I understand and agree that Polanie shall be under no obligation to use the above mentioned recording, in whole or in part.

(6). I acknowledge that this is a performing dance group and therefore understand that photos and videos may be taken and used for advertising.

(7). I agree and give permission that photos and videos in which the dancer appears in may be shared publicly.

I am of full age (18 or more) or am the parent of a dancer below the age of 18. I or my child has chosen to participate in dance instruction given by Polanie Dance Group of Ottawa. I acknowledge that I understand the nature of the activities the dancer will be participating in and the possibility that despite precautions, accidents and/or physical injury may occur. I hereby agree to the following:                                                                                 

 (1). That I or my child is participating in a dance class with Polanie Dance Group. I recognize that dance requires physical exertion which may be strenuous and may cause physical injury. I am also fully aware of the risks and hazards involved.                                                                                                                          

(2). I understand that it is my responsibility to consult a physician prior to and regarding participation in the class. I represent that I or my child is physically fit and have no medical condition which would prevent my full participation.                                                                                                                          

(3). I agree to take full responsibility for any risks, injuries or damages known or unknown which might incur as a result of participation in the class.                                                                                                                          

(4). I knowingly and voluntarily waive any claim I may have against Polanie Dance Group, or any volunteers, of injury that I or my child may sustain, or property damage that may occur as a result of participating in classes.

(5). I relieve Polanie, & representatives of all responsibilities, damages and losses arising from the dancer's participation.
