Notes 備註:
1. 請有意參加者於11/17(五) 17:00前完成報名。Please register before November 17th 17:00!
2. 說明會提供70元餐券1張，請於期限內於學餐指定店家使用。因會議室座位有限，報名人數以30名為限。A meal coupon (NT$ 70) will be provided; please use it at the CGU cafeterial within 1 week. Due to limited seating in the conference room, registration is limited to 30 people.
3. 參與者送出報名表後即表示同意將下列資訊授權長庚大學及日本台灣交流協會使用。After sending this registration form, paticipants agree to authorize Chang Gung University and the Fulbright Taiwan to use the following information.
(1)說明會中所拍攝及錄影之人像 Images captured and recorded during this information session.
(2)本報名表收集之個人資料 Personal information collected from this registration form.
4. 主辦單位保有最終修改、變更、活動解釋及取消本活動之權利。Chang Gung University reserves the right to amend or terminate the event.
