ATHLETIC RESPONSIBILITY AND REGISTRATION FORM 19/20
If you have more than one child participating in athletics you must register each one separately. If you are played another sport in the Fall or Winter season, you do not need to complete this again. The Swan Valley School District has instituted a pay-to-participate fee for athletics. The fee for the 2019-20 school year for athletes in 9th through 12th grades will be $100. The fee for the 2019-20 school year for 7th and 8th grade athletes will be $50. There is a separate registration for those grades. If an athlete qualifies for free or reduced lunch all fees will be reduced by 50%.  Family cap of $250.  Self-funded sports are excluded from the fee (Hockey, Boys/Girls Bowling, Sideline Cheer, Equestrian, Middle School Cross Country, Middle School Wrestling).
A STUDENT SHALL NOT PARTICIPATE IN ANY PRACTICE SESSION OR CONTEST UNTIL THIS ONLINE REGISTRATION FORM HAS BEEN COMPLETED AND THE SPORTS PHYSICAL HAS BEEN HANDED IN TO THE COACH. The physical must be dated on or after 4/15/19. If you participated in a Fall or Winter sport, you will NOT need to complete this process again.  

Due on first day of tryouts:
Completion of this online registration form with all of the required signatures from parent/guardian/athlete. Coaches will have access to be able to see who has completed the form.
Sports physicals will need to be turned in directly to the coach. Per the MHSAA, if the athlete does not have it, they will not be able to try out and/or practice. Physicals must be dated on or after 4/15/19 to be considered current.
Due second Monday of September (September 9):
$100 ($50 for 7th and 8th grade) yearly pay-to-participate fee. Check or money order to: Swan Valley School District.

I understand that the fee I am paying does not guarantee playing time or control over any conditions of the team, and is not refundable. I also understand that paying a fee does not alter Swan Valley Board of Education Student Policies, Michigan High School Athletic Association regulations, the District Athletic Code, and individual team rules.

CONSENT FOR TRANSPORTATION
I hereby give my son/daughter, permission to:
Be transported by bus to games, tournaments and competitions when possible.
Leave from athletic contests with a parent, relative, or authorized adult.
Accompany the team as a member on its out-of-town trips.
This authorization is in effect for the entire school year.
I understand that the ability of coaches and other school officials to properly supervise students may be impaired when students are not under their direct control. I agree that coaches should not be held accountable when students who are authorized to use alternative means of transportation do so. I understand that coaches reserve their right to refuse requests by players to leave their teams if, in the coaches’ opinion, it serves the best interest of the individual or the program. I agree to release the Swan Valley School District, its employees, and officers from all liability with reference to the above stated transportation.

EQUIPMENT FINANCIAL RESPONSIBILITY
Each athlete in the Swan Valley School District is responsible for the athletic equipment issued to them by the Athletic Department. This equipment is to be worn only for practice, coach designated demonstration, or school competitions in the sport for which it was issued. It is not to be worn at other times.
This equipment represents a large expenditure of money by the Athletic Department and is to be returned to the coach upon completion of the sport season (within one week). If the equipment is LOST, STOLEN OR NOT RETURNED, the athlete will be held responsible for the replacement cost of the equipment.

ATHLETIC CODE OF CONDUCT
Swan Valley School District firmly believes that, in order to have a successful athletic program, rules must be established that will allow athletes to perform to the best of their ability. Therefore, please review with your son/daughter our Athletic Code of Conduct. The Athletic Code of Conduct is available in the Athletic Office and at: www.swanvalleyschools.com  (Go to Athletics; Athletic Handbook). By electronically signing this online registration form, parents/guardians and students are acknowledging that they have read the Athletic Handbook.

NCAA CLEARINGHOUSE
It is the responsibility of the athlete and his/her parents/guardians to register with the NCAA Clearinghouse if he/she plans to play collegiate sports at the Division I or II level. The athlete and his/her parents/guardians is also responsible for ensuring that he/she meets the eligibility requirements for Division I or II participation. Register at www.eligibilitycenter.org .
