I have reviewed the GPMS Band Handbook and have read it, understand its contents, and agree to help enforce all of the expectations and policies set for in this handbook; I understand that my child’s failure to follow these may result in a lowering of his/her grade or a disciplinary consequence.
I have received a band calendar and understand that my child is committed to the events listed on it. I understand that I must help my child get to these events and that failure to so can result in a lowering of his/her grade. I understand that I am an important part of the band family!
I understand that for some events my child will need to ride a school bus. I hereby release Galena Park ISD and the band directors from any liability resulting from an accident involving my child while on a school bus. In case of emergency, I hereby authorize a District representative to seek medical attention for my child.
He revisado el Manual de Banda de GPMS y lo he leído cuidadosamente, entiendo su contenido y estoy de acuerdo en seguir todas las expectativas y políticas establecidas en este manual; comprendo que si mi hijo/a no sigue estas puede ocasionar que baje su calificación o que haya consecuencias disciplinarias.
He recibido un calendario de banda y entiendo que mi hijo/a está comprometido/a con los eventos anotados en éste. Comprendo que debo ayudar a mi hijo/a a llegar a estos eventos, y de no hacerlo puede ocasionar la disminución de su calificación. ¡Entiendo que soy una parte importante de la familia de banda!
Entiendo que para algunos eventos mi hijo/a tendrá que usar el servicio del autobús escolar. Por este medio libro a Galena Park ISD y a los directores de banda de cualquier responsabilidad resultado de un accidente que involucre a mi hijo/a mientras se encuentre en el autobús escolar. En caso de emergencia, por este medio autorizo al representante del Distrito para que busque atención médica para mi hijo/a.
