A form for Student Contacts (e.g. parents or guardians), available for submissions, at any time should they need to request a child's absence from School for exceptional circumstances.
PLEASE NOTE THIS IS NOT A FORM TO REPORT ILLNESS, OR MEDICAL/GP APPOINTMENTS
Please use the Studybugs app or absence line (01483 504172) to report your child's absence before 9.30am on the day of absence.
To report a future absence due to a medical or GP appointment, please email infants@stoughton.surrey.sch.uk with the date and time of the appointment, and include a copy of the appointment letter/text message.
