Use this form to formally request a refund for Hanes Field Trips you paid for but were cancelled due to NC and WS/FCS public safety statements. Please realize the magnitude of this event and resulting level of effort on school and district staff to process literally thousands of refund requests. It may take awhile.

Refunds will be issued by check and sent to the address you provide on this form (subject to verification).
