By typing my first and last name here, I understand that I am responsible for any materials borrowed with my library card. I also understand that I will become ineligible for this service if I do not return library materials. I further release the Northborough Free Library from any liability occurring as a result from participating in this program. Couriers or librarians will call to confirm a date and time for delivery. All deliveries will be made to the front door unless otherwise specified (use the space at the bottom of the form).  For the safety of our drivers, we will be unable to deliver items to a location other than the address provided. Couriers will not enter your home.  Couriers will not exit their vehicle in the presence of an unleashed dog. Couriers are only able to accept returns for library materials that are placed in a bag and are unable to accept donated items or any other non-library items. Couriers may not accept gifts or tips. *
