Critical Workers Child Care Registration

REGISTRATION DUE:  by Noon on Thursday, Dec. 7

NOTIFICATION:  Families will receive an email if their child (ren) are CONFIRMED in CWCC or have been placed on the WAITING LIST:   by 6 pm on Thursday, Dec. 7

Dover-Eyota Public Schools will be offering Critical Workers Child Care (CWCC). The district will provide no cost child care during school hours for students attending Kindergarten to age 12 for qualified households. Programming for CWCC is fluidly changing with changes and updates to the directives from the state.  D-E will continue to modify the CWCC program as directed.

See information regarding program registration, qualifications, employment, fees, dates, etc. on the email sent to families on 12/1/20.    

Child (ren) must be currently attending Dover-Eyota Schools in grades: Kindergarten - Age 12  

HOW TO REGISTER:
1. Complete this Google doc on-line Registration Form.

2. Complete the Google Form: Verification of Tier One Employment.  

Registration is FIRST COME FIRST SERVED for qualified families.

*You must fill out a separate registration form for each child.

Updated by Minnesota Department of Education on 11/5/20                                                                                                        “The school-age care for children of critical workers is intended for extreme circumstances in which no parent or guardian is at home, as all of the parents or legal guardians in the child’s household are Tier I critical workers. If it is a two-parent household and one is not a critical worker, a school is not required to provide free school-age care to the family, but the school could choose to provide the care, if they have the capacity to do so while ensuring health and safety requirements can still be met. School districts and charter schools may also encourage parents or legal guardians of Eligible Children to not utilize the school-age care program if they have the ability to work from home.”.  Type Your Name here verifying that you are not able to work from home and your employment status qualifies. *
Children must be well enough to attend care. Guidelines regarding when students may attend or are excluded from care are provided by Public Health and include both the child's symptoms and exposures to COVID19. It is absolutely imperative to stay home when experiencing COVID symptoms (e.g., fever, cough, chills, nausea, etc.) and while awaiting test results.  Type your name here stating that you will only send your child when they are healthy per our guidelines. *

By signing up for Critical Worker Child Care, I understand I am accepting the risks inherent in this activity and releasing Dover-Eyota Public Schools from all claims of injury that result from participation in care.
