To complete registration for any of the 2024 Multi-Arts Summer workshops you must:
•Complete this online registration and submit
•For new multi-arts families: Sign and return Release of Liability Agreement.
•Send certification of up-to-date immunizations/most recent physical exams. Include COVID immunization.

Before enrolling your child in our program, please note that we are unable to provide specialized or one-on-one care for individual children. While we will do our best to make our programs adaptive for all children, we are a small organization with limited staff and resources.

We retain the right to ask any child to be removed if they pose a threat to the well-being of themselves or others.

I give the Director and staff of Multi-Arts permission to administer minor first aid to my child. Should more than a minor injury occur, requiring emergency treatment, I give permission to the Director and staff to acquire emergency medical treatment (i.e. ambulance).
