PS 34 Student Learning Device Request  2022-2023
If you need a learning device for your child please complete the form and we will contact you when your device is ready for pick up.

Please note:

-All devices are LOANED to students and must be returned if the student leaves the Department of Education.

-If you no longer need the device or move from the school you are responsible for returning the device along with all plugs, wires and cases to the school. If you no longer need the device or move from the school you are responsible for returning the device and all plugs, wires and cases to the school.

-You will be contacted when your child’s device is ready to be picked up.

-Families must fill out the Student Device Loan Agreement upon pick-up.
