This form is to be used to request transportation for the 2020-2021 School Year for a Westerville City School (WCS) student who will be attending a Columbus Career Center while WCS is holding in-person learning according to our cohort A/B system (Scheduled to begin Monday October 26, 2020).

Before proceeding further please make sure that you have identified the following:
- The student's WCS Student ID number (starts with a 3).
- The student's WCS cohort identifier (A or B).
- The student's CCS cohort group identifier (Mon/Tue, Thu/Fri, or four day program)
