As an employee or responder for the County of Surry, I understand that I may learn or have access to information (verbal, written or electronic) which may contain protected medical details, criminal investigative information or other safety-sensitive and/or confidential information. By signing this agreement, you are being given access to Surry County Active CAD because of your affiliation with a volunteer department or as an employee of the County of Surry. I understand that I cannot share this access with any other person or entity in any way. I also agree to maintain the confidentiality of the information I am privileged to and not divulge it to anyone in anyway.

I understand and agree that any failure to comply with the confidentiality requirements set forth in this agreement is grounds for disciplinary action and permanent revocation of access to Surry County Active CAD. Additionally, the county may seek other criminal or civil sanctions and/or damages as allowed by law. If for any reason your access is terminated, the conditions of this agreement will apply beyond access or employment.

Surry County Active CAD should be accessed for work or response purposes only. Any information viewed should be specific to the accessing agency personnel and/or job duty. Surry County 911 Communications should be notified with 10 business days of any user who no longer needs access.

All user access will be monitored, and any unauthorized access will be investigated.

I have read and understand this agreement and agree to comply with it in every respect.
