I hereby affirm that this student is in good health and I have no knowledge of any physical impairment that would be affected by the participation in Summer Music Camp. I understand taht as a condition of admittance as a camp participant, I hereby release MTMS, Angie Eisenbarth, and all other employees of the camp from any liability resulting in injury or illness while at camp.
