Please use the following form to submit answers to continuing education questions found in certain issues of the Refresh Magazine. Each completed submission is worth 1 CEU. Only 1 CEU per issue of Refresh will be awarded.

PLEASE NOTE that as of January 1, 2022, we will no longer be automatically logging these credits. This means that you will need to add these activities to your log sheet but you will not need to supply any supporting documents as we are able to check whether or not you have submitted your answers.
