Please make sure all information provided below is correct so that your swag makes it to you.

I am currently only able to offer swag to US residents due to shipping prices.

Once the form is filled out, I will add your name to my waitlist. As I reach names on the list, I will be sending an invoice to you. You will have 24 hours from the time it's sent to pay. If you do not pay, I will cancel your order and you will have to resubmit.

If you want a peen, please notate up to 2 colors you'd like in your peen(s).

Colors notated below will be used in all peens ordered on this order form. If you are wanting peens in different colors, you will need to submit separate order forms. Please notate on the order form that you are ordering a set with different colors so that I know to watch for your other order forms.
