I as the parent/guardian of the applicant, (name of applicant that you will enrol in) wish to enrol the applicant in the Saints Rugby Football Club (Saints RFC), a subgroup under St Andrew’s Alumni (SAA) established in Singapore and hereby give my consent to his/her application as a Saints Junior Rugby Player to be trained under Saints RFC. I declare that:
a) the applicant and I will participate in Saints RFC activities at our/his/her own risk;
b) neither the applicant nor I will hold SAA, Saints RFC or any of its Members, officers, agents, contractors or guest coaches liable for:
i. Any injuries suffered by me;
ii. Any injuries suffered by the applicant or by any other person; or
iii. Any loss of, or damage to, any of the applicant’s, mine or any other person's property, in connection with the applicant’s and/or my participation in the activities under Saints RFC, howsoever caused and expressly including but not limited to any act, default of Saints RFC or any of its Members, officers, agents, contractors or guest coaches;

c) the above information is true and accurate; and
d) the applicant and I will abide by the Saints RFC rules and any bye-laws, rules or regulations promulgated by SAA and Saints RFC from time to time.
e) the applicant and I will at all times act in a manner that is appropriate in representing the Saints RFC and shall endeavor at all times to participate in the activities of Saints RFC demonstrating good sportsmanship and behaviour to team mates, match officials and opponents.

Personal Data Protection Act

1. In compliance with the Personal Data Protection Act (“PDPA”), St Andrew’s Alumni (SAA) and Saints Rugby Football Club (SRFC) seek your consent (“as parent and/or guardian”) to collect and use your child's personal data such as names, videos and photographs taken during training, games and activities organized by SAA, Saints RFC and their affiliated organisations where necessary (“Purposes”).

2. SAA and Saints RFC will also collect and use yours and your child's personal data to provide you with information and newsletters on membership activities such as upcoming events, training, games organised by SAA, Saints RFC and affiliated organisations which may be relevant to you (“Services”). Photographs, quotes, names of members and activities may be posted on our public access medium (including but not limited to posters and on Facebook).
