Please fill out this form to begin the ordering process. Kindly fill out a new form for each DIFFERENT item being ordered. (Multiple quantities of the same item can be indicated on one form). Once your order has been submitted, I will send you an invoice within 24 hours. Orders will not be started until the 50% deposit has been paid. Custom orders cannot be returned or refunded.
