 I/We confirm that the details contained in the form are correct and that by submitting this form we are giving/withholding consent as indicated on the form. I/We note that the pupil cannot be enrolled unless this form is completed.  I/we confirm that we have parental responsibility for the pupil named on this form.

I/We note that the Enrolment process will not be complete until the Form of Acceptance, signed by both parents/guardians,  and copy of Birth Certificate have been submitted to the School.
