After completing this form, you will receive an automated email with all of the dates that you signed up for hot lunch. Please keep this email to refer back to throughout the trimester.  You will receive this email within one minute of filling out the form. You may need to check your spam folder. 

You will be billed for the whole trimester through Procare on February 16th with invoices due on the 20th. 

You only need to choose an option for the days that you would like to sign up. If no option is recorded, it will be perceived as not having hot lunch that day.
