10/18 Haven CCC In-Person Worship Reservation
Please use this form to make a reservation to join us in-person for the 10/18 worship service.  You do NOT need to fill out this form to join us online.  

Space is limited to allow for social distancing.  We will take reservations on a first-come-first-served basis.  Our apologies if we are not able to fit you in this week.

Note: we require your email address so that we can notify you when we confirm your reservation (normally within 24hrs of you filling out this form).  We may also use it to contact you in case there are updates to the service.

For safety reasons:
1) Please do not show up for in-person services if you do not receive a confirmation email.  We need to enforce this for social distancing purposes and will not be accepting unregistered walk-ins.
2) If you are feeling ill Sunday morning we ask that you stay home and worship online.

* Additional Safety Precautions for In-Person Services
1) Everyone must wear their mask at all time.  
2) Everyone must be 6 feet social distancing in compliance at all times.  
3) Hand shaking, hugging and fist bumps is temporarily restricted.
4) Attendees will bring their own chairs, water and Bible.
5) There will be no passing out of paper handouts (i.e. if you would like to refer to the sermon outline, please print it at home before you come)
6) Attendees will be ushered in by a designated usher to the specific assigned area. Larger areas will be designated for families with children.
7) Attendees are not able to wander or congregate with others before or during the service.
8) Hand sanitizer will be available.
9) Attendees are asked to leave immediately following the service.
