The person completing this form must enter their email address as the primary contact. All email correspondence and information can only be sent by us to the primary email address provided below. If Parent/Carer 2 requires a copy of any information sent, we ask for the person completing this form to forward this information to them. Please note: Upon a successful application, and until entry to the school in September, this will be the only email address that the school will use for admissions correspondence.

If you make or have made an error in your application, or are not sure if you have already submitted an application to us, PLEASE DO NOT SUBMIT A SECOND SUPPLEMENTARY INFORMATION FORM.  Please telephone the Admissions Officer on 01604 230240 who will be happy to make amendments for you, or check that we have your application.
