TECH RESPECT: RESPONSIBILITY STARTS WITH “U”
Effective Monday, January 24, 2022,  the Technology Department  and the Office of Teaching and Learning will launch our new TECH RESPECT: RESPONSIBILITY STARTS WITH “U” Chromebook campaign. This district-wide effort is designed to promote and reinforce student responsibility.

THE PURPOSE OF THIS FORM IS TO NOTIFY YOUR STUDENT'S SCHOOL THAT YOU DO NOT GIVE YOUR CONSENT FOR YOUR STUDENT TO RECEIVE A REPLACEMENT CHROMEBOOK CHARGER FOR A COST OF $25.00 EACH.  

PLEASE COMPLETE A FORM FOR EACH STUDENT THAT CURRENTLY ATTENDS A CM 201-U SCHOOL.
