This screening attestation is required by all members, players, volunteers and coaches prior to entering the Crusaders Rugby Club facilities and MUST BE COMPLETED A MAXIMUM OF 4 HOURS before arriving at the club.

If you have a digital Membership Card on your phone, we can scan that to verify that you have completed this form. Alternatively, please bring a printed copy of the email confirmation you receive after the completing this form.
