Please complete the following items that are required as part of your Stage II Application. Before completing this application, you must go to http://www.atu.edu/teachereducation/ and familiarize yourself with this information particularly as related to the policies and procedures and the dress code that you will be expected to follow as a pre-service professional in education. Keep in mind that you must complete each of the starred questions below for your application to be processed in a timely manner. If you do not complete each of the starred questions, you will be required to redo the entire form for processing.

Within this second part of this application, you will be asked to answer questions pertaining to your Praxis Content Area Assessment located at: https://www.ets.org/praxis/ar/requirements. You will answer questions from this site pertaining to your assessment, and you will be answering questions from the study companion provided there for your respective assessment. It would be good to have this site available as you answer these questions.

You will also be asked to provide two to three paragraph responses for two questions. One involves why you chose to become a teacher, and the second involves what an innovative teacher looks like in today's day and age. You will be asked whether you have reviewed the ATU Policies and Procedures Manual located at https://www.atu.edu/teachereducation/. Finally, there is an ATU Media Waiver to examine on the the previously mentioned Teacher Education website. Please examine this. It would be good to consider each of the aforementioned aspects before beginning this survey, so you will be able to answer the items pertaining to these aspects.
