Teen Subscription Box Request Form
Youth ages 10-18 years old can register to get a once a month subscription box full of fun activities!

What's in the box?
1.) A creative, fun activity with all the supplies included.
2.) A library book, already checked out to you, chosen for you based on your answers to questions on this form.
3.) A special treat such as tea, candy, popcorn, etc.  Please let us know if you have any allergies or restrictions, so we can customize your box. (I don't want your orthodontist mad at me!)
4.) A survey, asking you to rate the book we selected.  That will let us know how we did in choosing your book.  Didn't like it? Be honest.  It will help  us select a book next month for you!

When and how do I get my box?
We will contact you when your box is ready to pick up.  Pick it up during regular library hours.  Just let library staff know you're picking up your subscription box. When you are done with the book, return it to the book drop!
