This form is to be completed each day and submitted a minimum of TWO hours before a practice or event. If at any time a student or coach has a fever, COVID-19 symptoms or test positive for COVID-19. The person MUST report it to the high school COVID Response Team. Anyone with symptoms must be tested for COVID-19 and the results MUST be shared with the school prior to allowing the person to return to the athletic team for practice and competitions. PLEASE REPORT SYMPTOMS AND KEEP EVERYONE SAFE AND HEALTHY.
