Breakfast Club Summer 2 2023 Booking Form

Please use this form to book your children into Breakfast Club.

Breakfast club runs from 7.45am each morning. The cost including breakfast is £4.50.

You may use a single form for all of your children if they are all attending the same sessions. If you require different sessions for different children, please submit a separate form for each child.

If your plans change after you have booked, we cannot accept changes made verbally. You may inform us of the change by resubmitting this form or by sending an email to asc@caton.lancs.sch.uk.

Please note, any changes made after 2pm on Friday of the week before the sessions are required will still be charged to your account.
