Changes will not be made after this form is sent, and I do not accept duplicate forms (multiple forms for the same order). If you have any questions, please ask before submitting. I may not accept questions/changes after this is submitted. This is the only way orders for these will be taken, so be sure in your choices! ♡´･ᴗ･`♡

Your email is collected in case there are issues with your form. Google will send you a receipt of your form responses to this email, as well, so please make sure it is correct. Your email and any other information is not shared whatsoever or used aside from these purposes.

Note: Your responses to the form will be deleted after I print them. If you have bought more than one case in a single order, please include information for all cases in one form. (◠‿◠✿)
