OUT-OF-SEASON ATHLETICS REGISTRATION FOR ACTIVITIES FROM: JUNE 1, 2022 - AUGUST 20, 2022

This form is to sign up for any SJCI athletic department out-of-season activities during the time period listed above.

Information about these activities will be posted on the SJCI Athletics website (click the individual sport and the TEAM INFO or PRACTICE SCHEDULE tab for information).

NOTES:
* You will still be required to register for an in-season sport prior to the official start date of the season.
* Not all SJCI athletic programs offer out-of-season training activities during this assigned time period.
* All participants must be a registered current or incoming student in good academic & disciplinary standing.

PHYSICAL:
Submit your physical directly to the SJCI nurse Rosanne Urbanski at rurbanski@sjci.com or by sending via mail to St. Joseph's Collegiate Institute / 845 Kenmore Ave. Buffalo, NY 14223 (Attention: Rosanne Urbanski). Copy and paste the link below to access the school physical form (note this form is not required as your PCP may use a different form that will also be accepted).

* Submitting this form acknowledges that you are participating in voluntary activities that you are medically cleared to participate in. All participants are expected to follow the SJCI student handbook and SJCI athletic department standards at all times.
