This form is used to collect parent email addresses for Edith Cavell Elementary PAC class lists. This information will be shared with the volunteer Class Parent(s), who may contact you about class gifts for the teacher, meeting minutes, or other items that are relevant to your child's class. These lists are only used by Class Parents, and are not distributed to parents for personal use (such as arranging playdates, etc).
Please make sure that you fill out the correct form (by division) for each of your children, as each list is now separately maintained.
